Any other professional services you may request will be billed at our agreed upon hourly fee and prorated for periods of less than one hour. Other services may include: report writing, telephone conversations longer than 15 minutes, attendance at meetings with other professionals you have authorized, preparation of records or treatment summaries, and the time spent performing any other service you may request of me.
I ask that you come to each session prepared with a credit or debit card for payment through IvyPay.
For clients with out-of-network insurance, payment in full is is due at the time of service. I will provide a monthly superbill to submit to insurance for out-of-network reimbursement. You are responsible for understanding your particular policy and confirming what your out-of-network benefits may be.
Common Services Include:
90791: 55min Intake Session ($220)
90834: 45min Individual Psychotherapy Session ($165)
90847/46: 50min Family Psychotherapy Session ($205)
90837: 55min Individual Psychotherapy Session ($205)
Sample Good Faith Estimate:
The sample estimate below is the amount you would owe if you were to attend therapy for 52 sessions in a year (weekly, without skipping any weeks for holidays, vacation, unplanned events/sickness, etc.):
[2x 90791 Intake Sessions at $220 each] +
[50x 90834 Psychotherapy Sessions at $165 each] =
$8,690/year
This is just one example to give an idea of financial expectations for a calendar year. Frequency and duration will be dependent on your individual needs and goals.
If you need to cancel or change an appointment, you must notify me no less than 24 hours in advance. If you cancel less than 24 hours before your appointment or do not show up for your scheduled appointment, you will be charged a cancellation fee equivalent to your session fee.
It is important to remember that insurance will not pay for missed appointments, so you will be responsible for the full fee, not just a copay.
This cancellation policy reflects the reality that your appointment time is reserved specifically for you and even if you are unable to attend, you are still responsible for the time.
Therapist Cancellations/Rescheduling
I will let you know with as much advance notice as possible should I encounter a scheduling issue or emergency. When this happens, you, of course, will not be charged.
In the rare event that I must cancel or reschedule a session with you, I will do my best to offer an alternate time to meet whenever possible. If this occurs and you notice yourself struggling with your feelings related to the cancellation, please know that they are a welcome part of our work together and that you are encouraged to bring them into session for discussion and processing.
